About The Position

Requirements

  • License and current registration to practice as a Registered Professional Nurse in the State of New York.
  • Minimum of 1 to 2 years experience as a registered nurse in Medical/Surgical nursing or home care.
  • Ability to assess and respond to the needs of patients and families in the home setting.
  • Professional Liability Insurance.
  • Ability to drive with an automobile in good working order and available for use in the field.
  • Basic computer skills.

Responsibilities

  • Conduct initial assessments and reassessments of patients.
  • Supervise Home Health Aides (HHAs).
  • Assess patient/family situations for nursing and health-related problems.
  • Provide nursing care to patients requiring part-time or intermittent professional nursing care at home.
  • Develop and coordinate individualized care plans in collaboration with health team members.
  • Instruct and supervise patients, caregivers, LPNs, and home health aides in patient care.
  • Report significant conditions/symptoms to the patient's physician and modify care plans as needed.
  • Initiate appropriate preventive and rehabilitative nursing procedures.
  • Educate patients/families on disease processes, self-care techniques, and prevention strategies.
  • Evaluate the effectiveness and outcomes of care provided.
  • Consult with and refer to other professionals or agencies regarding patient needs.
  • Interpret agency policies, procedures, and regulations to patients/families.
  • Document all nursing services provided and maintain compliance with regulations.
  • Plan for discharge from service systematically.
  • Manage document flow and confidentiality of patient records according to agency policy.
  • Adhere to agency policies and state/federal regulations governing home care services.
  • Participate in program evaluation, research, and educational programs.
  • Maintain sufficient service productivity to meet agency goals.
  • Engage in professional growth activities.
  • Participate in the orientation of new staff and student nurse programs.
  • Perform other duties as assigned.
