About The Position

The Diabetes Educator Specialist (RN) at Park Nicollet Clinics is responsible for providing diabetes self-management care and education to patients, either individually or in groups. This role serves as a primary resource for the interdisciplinary team, focusing on assessment, education, and management of diabetes care. The position is on-call and requires a commitment to improving patient care and experience.

Requirements

  • Bachelor's degree (BA, BS, or AD) in Nursing.
  • Minimum of 2 years of experience as a Registered Nurse (RN).
  • Current RN licensure in the state of Minnesota.
  • Current national certification as a Certified Diabetes Care & Education Specialist (CDCES) or ability to obtain within 24 months of hire.
  • BLS provider status in accordance with American Heart Association (AHA) guidelines.

Nice To Haves

  • Five years of experience as an RN is preferred.
  • Strong assessment and problem-solving skills.
  • Ability to understand and apply teaching/learning principles.
  • Effective communication skills.
  • Ability to work independently while supporting team members.

Responsibilities

  • Provide individual or group diabetes self-management care and education for patients with diabetes.
  • Serve as the primary resource for the interdisciplinary team in assessment, education, and management of diabetes.
  • Demonstrate knowledge of age-related growth and development principles necessary for patient safety.
  • Ensure compliance with laws, regulations, and internal policies related to patient care.
  • Utilize various glucose monitoring technologies and insulin delivery systems effectively.
