About The Position

The Licensed Vocational Nurse (LVN) position at Kaweah Health involves providing comprehensive patient care in a skilled nursing or acute rehabilitation environment. Requirements

  • CA Licensed Vocational Nurse certification required.
  • BLS certification required.
  • Intravenous therapy certification and blood withdrawal certification within six months of hire/transfer required.
  • Experience in a skilled nursing or acute rehabilitation setting preferred.
  • Six months experience within the past two years working in a general acute care facility or equivalent experience appropriate to the type of subacute patient the facility provides care for.

Responsibilities

  • Carries a full patient assignment in the skilled nursing or acute rehabilitation environment.
  • Has overall responsibility for the patient care needs of each assigned patient during their shift.
  • Uses and practices basic assessment (data collection).
  • Observes assigned patients, charting and reporting changes in patients' conditions and taking any necessary action.
  • Measures and records vital signs, intake and output.
  • Performs point of care testing, etc.
  • Provides basic patient care and treatments.
  • Assists patients with activities of daily living and mobility needs.
  • Administers prescribed medications or starts intravenous fluids and administers blood products.
  • Collects samples, administers skin tests and vaccinations.
  • Participates in planning, executes interventions in accordance with the treatment plan and contributes to the evaluation of nursing intervention outcomes, conferring with other health care team members as necessary.
  • Collaborates and communicates with the patient/resident, family, and other health care providers.
  • Contributes to the development and implementation of a teaching plan related to self-care for the patient.
  • Acts as a patient/resident advocate under the direction of the RN.
  • Makes referrals, including provisions for continuity of care, as needed.
  • Assists in development and implementation of policies, procedures, and practice guidelines to improve quality of care and nursing practice.
  • Participates on interdisciplinary teams.
  • Maintains patient confidentiality and privacy within legal and regulatory parameters.
  • Delivers care in a nonjudgmental and nondiscriminatory manner that preserves patient autonomy, dignity and rights.
