The Intake and Service Coordinator plays a crucial role in providing Non-medical Case Management, Health Education/Risk Reduction, Food Bank/Home Delivered Meals, and Psychosocial Support Services to Ryan White-eligible populations, particularly focusing on BIPOC individuals. This position is responsible for assessing client needs, developing care plans, and ensuring access to essential services, while also participating in program recruitment and compliance with funder requirements.

  • Organizes and facilitates client eligibility assessments and centralized intake processes.
  • Evaluates ongoing operations and ensures compliance with funder requirements.
  • Conducts quality assurance audits of client charts to ensure service quality.
  • Participates in data collection and continuous quality improvement efforts.
  • Provides educational resources and engages potential clients in programs.
  • Presents Iris House services to community organizations to generate referrals.
  • Monitors and evaluates care plans and staff training needs.
  • Identifies opportunities for quality management improvement in service delivery.
  • Maintains a workplace that values diversity and is free of discrimination.
  • Bachelor's Degree in Human Services or related field or equivalent experience.
  • At least five years of related experience, including two years of management experience.
  • Knowledge of HIV/AIDS prevention, food security, and case management.
  • Strong group facilitation and communication skills.
  • Proficiency in personal computer use.
  • Experience with Ryan White programs.
  • Spanish bilingual preferred.
