About The Position

The Hospice Unit RN/LPN position at Pikes Peak Post Acute involves providing compassionate care to patients in a hospice setting. Requirements

  • Current, unencumbered, active license to practice as an RN/LPN in Colorado.
  • Wound Care experience required.
  • 6 months of hospice experience.
  • 2 years or more experience in a long-term care facility preferred.
  • CPR certification.
  • Ability to pass a criminal background check and Colorado CAPS background check.
  • COVID vaccine required or valid exemption.
  • LPNs must be IV certified.

Responsibilities

  • Accurately observe and assess wounds and pressure sores, notifying physicians of any lack of progress or deterioration.
  • Ensure appropriate wound care management services are included in care plans to promote healing.
  • Assist in preparing and updating care plans for treatment-related issues.
  • Assist physicians and other healthcare providers during visits and examinations.
  • Communicate with physicians and health professionals regarding resident care and treatment.
  • Notify family and attending physicians of significant treatment-related issues.
  • Participate in the orientation and ongoing training of nursing staff.
  • Report significant findings or changes in patient condition to the LPN Supervisor or Director of Nursing.
  • Monitor patient's condition and assess their needs for optimal care.
  • Observe and interpret patient's symptoms and communicate them to physicians.
  • Collaborate with healthcare teams to devise individualized care plans for patients.
  • Perform routine procedures such as blood pressure measurements and administering injections.
