About The Position

The Hospice Aide provides essential personal care services to terminally ill hospice patients under the supervision of a registered nurse. This role involves assisting patients with daily living activities, offering emotional support to both patients and their families, and ensuring adherence to care plans. The aide plays a crucial role in the hospice team, contributing to the overall well-being of patients during their end-of-life care.

Requirements

• High school diploma or GED.
• Graduate from an approved training and competency course.
• Must be a Certified Home Health Aide.
• Minimum 2 years of experience as a nursing assistant, preferred.
• Active patient contact within the past three years, preferred.
• Ability to work independently and make accurate judgments.
• Ability to respond appropriately to crises outside of a hospital setting.
• Acceptance of and adaptability to different social, racial, cultural, and religious modes.

Responsibilities

• Provide personal care assistance to hospice patients under the direction of the RN.
• Assist with bathing, mouth, skin, and hair care.
• Support patients in using the bathroom or bedpan.
• Facilitate bed transfers and ambulation activities as per care plan orders.
• Perform range of motion exercises as taught by professional personnel.
• Carry out incidental household services essential to the patient's well-being at home.
• Prepare meals, including special diets as needed.
• Monitor and record vital signs such as temperature, pulse, respirations, and blood pressure if assigned by the RN.
• Assist with shopping for patients without other resources.
• Provide emotional support and compassionate care to patients and their families.
• Document patient care and report any changes in condition to the RN.
• Participate in annual performance reviews and maintain required competencies.
