About The Position

The Clinical Liaison is a registered RN/LPN responsible for promoting and increasing the use of Good Shepherd Hospice within assigned nursing facilities. This role emphasizes ethical practices, customer service, and effective communication to ensure eligible patients receive timely hospice care. The Clinical Liaison collaborates with healthcare professionals, manages admissions, and develops growth strategies while adhering to compliance and ethical standards.

Requirements

• Must have a current RN/LPN license in the state of the program assigned and be in good standing.
• Must have a valid driver's license and an acceptable driving record.
• Thorough knowledge of sales techniques and market analysis.
• Commitment to the hospice philosophy of care.
• Proficient in financial analysis and technology (Word, Excel, PowerPoint).
• Extensive experience in consultative sales and customer service.

Responsibilities

• Work closely with identified referral sources in all assigned facilities to ensure timely hospice care for eligible patients.
• Assist with admission planning by coordinating with patients' physicians and nursing facilities regarding evaluations and care plans.
• Utilize required technology daily and weekly for communication and record-keeping.
• Maintain established contact and education averages set by the Director, including follow-ups with referral sources.
• Provide customer service and informational visits to patients and families.
• Assist in developing market and company-wide growth strategies.
• Achieve and exceed goals set by the Directors.
• Ensure positive engagement between sales and clinical members of the hospice team.
