About The Position

The Aging and Disability Medicaid Case Manager II is responsible for conducting in-home assessments and arranging caregiving services for elderly and disabled clients receiving Medicaid-funded services. This role focuses on ensuring clients receive culturally appropriate support to maintain their quality of life while aging in place. The position requires collaboration with clients to develop and implement care plans that address their daily living needs.

Requirements

• Bilingual ability in Arabic preferred.
• Experience working with aging and disabled populations.
• Ability to navigate a dynamic and flexible work environment.
• Strong communication and interpersonal skills.

Responsibilities

• Conduct in-home comprehensive functional assessments for elderly and disabled clients.
• Arrange for in-home caregiving services for clients receiving Medicaid-funded personal care.
• Develop and implement care plans to ensure clients' Activities of Daily Living are met.
• Collaborate with clients and their families to understand their needs and preferences.
• Advocate for equitable access to services for low-income, immigrant, and refugee populations.
