The Ambulatory Clinical Oncology Pharmacist plays a vital role within the oncology care team at Southern New Hampshire Medical Center, delivering expert clinical pharmacy services to providers, nurses, and patients in the outpatient oncology setting. This position focuses on ensuring the appropriateness, effectiveness, and safety of cancer treatments, while also promoting cost-conscious care and supporting patient adherence to therapy. Additionally, the pharmacist is responsible for managing adverse drug events and addressing coexisting medical conditions to optimize patient outcomes.

  • Monitor treatment responses, adjust medications, and order labs in collaboration with providers.
  • Educate patients and families on medication use, side effects, and interactions.
  • Conduct medication therapy management and review for drug interactions or contraindications.
  • Collaborate on individualized care plans and participate in interdisciplinary teams, including the Cancer Committee.
  • B.S. in Pharmacy or PharmD.
  • Current licensure in the State of New Hampshire or eligible for licensure.
  • Minimum two years of ambulatory oncology pharmacy experience required.
  • Board Certified in Oncology Pharmacy (BCOP) Preferred.
