About The Position

The Marion, Illinois VA Medical Center is seeking a Social Worker to join the Care Management & Social Work Services as an Advance Care Planner. This role involves facilitating Advance Care Planning group visits and providing comprehensive social work services to Veterans and their families, particularly during times of crisis. The Social Worker will collaborate with interdisciplinary teams to assess psychosocial needs, develop treatment plans, and provide ongoing support and education regarding available services and resources.

Requirements

• U.S. Citizenship is required.
• Master's degree in social work from an accredited school of social work.
• Licensure or certification to independently practice social work at the master's level is required for GS-11 and above.
• Proficiency in written and spoken English is mandatory.

Nice To Haves

• Experience as a Social Worker in inpatient, outpatient, or home health settings.
• LCSW licensure is preferred.
• Two years of case management-related duties.
• Prior experience working with an interdisciplinary team.

Responsibilities

• Facilitate Advance Care Planning group visits and educate Veterans about the process.
• Conduct assessments of Veterans in collaboration with the interdisciplinary treatment team.
• Develop treatment plans and set achievable goals with Veterans and their families.
• Provide short-term, solution-focused counseling to Veterans and their caregivers.
• Make referrals to appropriate services based on Veterans' preferences and needs.
• Educate Veterans and families about VA and community resources, entitlements, and Advance Directives.
• Provide ongoing supportive counseling to help Veterans cope with chronic or catastrophic illnesses.
