About The Position

ADAPT Community Network, formerly United Cerebral Palsy (UCP) of NYC, provides a multitude of services to people with developmental disabilities. The Social Worker will counsel consumers and their families, assisting them in understanding illness and disabilities while helping to resolve personal, financial, and environmental difficulties that interfere with obtaining maximum benefits. This position works in accordance with ADAPT policies, regulatory agency requirements, and professional standards, developing collaborative relationships with other disciplines to ensure proper client care. This is a part-time position requiring 30 hours per week, with flexible scheduling.

Requirements

  • Master’s Degree in Social Work from an accredited program
  • One year of prior experience as a Social Worker preferred.
  • New York State Certification (CSW) or equivalent.

Responsibilities

  • Participates in intake process and develops initial and annual social service summary on each person receiving services.
  • Develops a treatment plan and goals based on information in summary.
  • Provides social services as prescribed by treatment plan in areas of counseling and concrete services including housing and residential placement, food stamps, Medicaid, half-fare cards, camp, vacation referrals, etc.
  • Integrates therapeutic intervention techniques into programs or home of person served through consultation with parent/guardian and staff at meetings, case conferences or through other informal contacts.
  • Participates in interdisciplinary team meetings and annual reviews as required.
  • Records progress on all active cases, monitors status and makes appropriate recommendations as changes occur.
  • Assists with arrival, dismissal, feeding, toileting as needed.
  • Directs clients/families to appropriate community agencies and resources for financial assistance, counseling, auxiliary care, adaptive devices, etc.
  • Keeps abreast of changes in field.
  • Safeguards information in relationship to individuals receiving services, including clinical, program and family information.
  • Trains and instructs staff in utilization of goals, rights of persons receiving services, confidentiality, family interaction, etc.
  • Provides supervision and training for Social Work Assistants and less experienced Social Workers.
  • Makes regular home visits to families on case load.
  • Attends professional meetings and conferences.
  • Performs other related duties as requested.
