About The Position

Broad Reach Healthcare is seeking a dedicated full-time Social Worker to join the team at Liberty Commons, a locally owned Skilled Nursing Facility in Cape Cod. This role focuses on providing personalized and compassionate care to residents, ensuring their well-being while also supporting the professional growth of staff. The Social Worker will work closely with the Director of Social Work on discharge planning and documentation, gaining hands-on experience in both short- and long-term care settings.

Requirements

• Minimum requirement of LSWA or above.
• Must hold a valid Massachusetts Social Worker’s license.
