About The Position
As an integral part of Rice Memorial Hospital Care Management team, the Social Worker's focus is to provide professional, medical social work services for hospital patients/families in assigned inpatient and outpatient departments. The individual in this position practices medical social work focusing on patient's/family's psychosocial needs through assessment, discharge planning, patient/family education regarding available resources, advocacy, support and linkage to community referrals. You will represent Rice Hospital on community boards, task forces and other activities assigned by the director, and be responsible for program development, evaluation and improvement in assigned clinical area. This position will work in the Cancer Center, Rehab department and Inpatient Behavioral Health.
