Under the supervision of the Director of Specialized Health and Social Services, and with oversight by the Regional Team Leads/Supervisors, provides advanced and comprehensive case management consultation for families with children who have health risks or special health care needs. Attends work on a regular and predictable schedule in accordance with agency leave policy and performs other duties as assigned. Oversees case management services for families in the Children with Special Health Care Needs Services Program, those that are on the CSHCN Program/Waiting list and non-Medicaid eligible, pregnant women on Title V or unfunded, children on the CHIP and CHIP Perinatal program, children referred by the Newborn Screening program, children referred by the Childhood Lead Poison Prevention program, children with a lapse in Managed Care Organization or Medicaid coverage/dis-enrolled, children carved out of the MCO service delivery model, children that are on traditional Medicaid, STAR Health clients remaining in Personal Care Services Program (PCS), and Children referred from the MCO’s who are in need of school advocacy, clients on the Personal Care Services (PCS) program, Department of Family and Protective Services (DFPS) and Case Management for Children and Pregnant Women (CPW) programs. Assists families with eligibility determination for state services and conducts comprehensive assessments to determine the need for PCS/PDN, CSHCN, and CPW services. Travels to conduct home visits. Completes Intakes, Family Needs Assessments, Service Plans, and follow-up to families. Links families to community resources. Communicates program services in a positive and accurate manner. Works as a team member. Participates and advocates in Community Resource Coordination Groups (CRCG), case management coalitions, interagency groups and community organizations. Provides information and referral services. Completes administrative duties, monthly reports, special assignments and work activities in a professional and timely manner. Employee participates and/or serves in a supporting role to meet the agency’s obligation for disaster response and/or recovery or Continuity of Operations (COOP) activation; such participation may require an alternate shift pattern assignment and/or location. Works under minimum supervision with considerable latitude for the use of initiative and independent judgment. Maintains confidentiality.

Requirements

  • Knowledge of social work and case management principles and best practices.
  • Knowledge of children’s programs such as Medicaid and Early Childhood Intervention, along with community resources on a local, state and national level.
  • Knowledge of cultural and socioeconomic issues related to health and disability.
  • Knowledge of maternal, infant and children’s health, to include mental health.
  • Skill in developing and presenting information to diverse audiences.
  • Skill in use of electronic mail, internet, databases, and Microsoft Office products.
  • Skill in problem solving; planning, prioritizing, decision making and organizing workload.
  • Ability to communicate effectively, verbally and in writing.
  • Ability to adhere to the National Association of Social Worker’s Code of Ethics.
  • Ability to provide own transportation to perform work duties and conduct home visits.
  • Current unrestricted Licensed Social Worker in the State of Texas (LBSW, LMSW, LMSW-AP, or LCSW).
  • Valid Class C Texas Driver's license or equivalent license from another state.
  • Excellent written communication as observed by detail and completeness of information provided on the State Application.
  • Willing to travel minimum of 65%.

Responsibilities

  • Provide advanced and comprehensive case management consultation and services for families with children who have health risks or special health care needs across various state and federal programs.
  • Oversee case management services for families in programs such as CSHCN-SP, Title V, CHIP, Newborn Screening, Childhood Lead Poison Prevention, MCO/Medicaid, STAR Health PCS, DFPS, and CPW.
  • Assist families with eligibility determination for state services and conduct comprehensive assessments for PCS/PDN, CSHCN, and CPW services.
  • Conduct home visits for comprehensive assessment of home environment and needs.
  • Complete Intakes, Family Needs Assessments, Service Plans, Personal Care Services Assessment forms, and follow-up services.
  • Link families to community resources, family supports, and advocacy.
  • Communicate program services accurately and positively.
  • Work as a team member.
  • Participate and advocate in Community Resource Coordination Groups (CRCG), case management coalitions, interagency groups, and community organizations.
  • Provide information and referral services.
  • Complete administrative duties, monthly reports, special assignments, and work activities in a professional and timely manner.
  • Maintain documentation of work activities, including record documentation, travel reports, case summaries, and correspondence.
  • Utilize software programs for database entry, word processing, spreadsheets, electronic mail, and Internet.
  • Participate and/or serve in a supporting role for agency disaster response, recovery, or Continuity of Operations (COOP) activation.
  • Maintain confidentiality.
  • Conduct reassessments for PCS/PDN and Case Management as required.
  • Participate and collaborate with other community entities on health, social work, and case management issues.
  • Serve as a DSHS representative on CRCG, coalitions, interagency groups, and advocacy groups.
  • Provide educational program presentations to the public, social service agencies, and HHSC programs.
  • Attend training and conferences to meet licensing requirements and develop professional knowledge and skill.
  • Maintain Social Work Licensure.
  • Participate in performance evaluations process and attend staff meetings.
