The social worker coordinates psychosocial and community resource needs in collaboration with the Hospice and Palliative Care Interdisciplinary Group (IDG). The hospice and palliative Social Worker demonstrates critical thinking, analysis, and clinical judgment in all aspects of hospice and palliative care delivery for patients and families experiencing life limiting illness.   • provides psychosocial support and counseling for patients and families facing life limiting illness
  • coordinates psychosocial and community resource needs in collaboration with the Hospice and Palliative Care Interdisciplinary Group (IDG)
  • demonstrates critical thinking, analysis, and clinical judgment in all aspects of hospice and palliative care delivery for patients and families experiencing life limiting illness
  • addressing the social, emotional, financial, and spiritual needs of patients and families to improve quality of life and reduce suffering
  • One year social work experience in health care setting preferred.
  • Master's Degree from an accredited School of Social Work.
  • Current licensed Social Worker in the state of Pennsylvania required.
  • Valid driver's license and automobile insurance.
  • Current CPR certification
  • This position requires you to wear a respirator as part of the job requirement.
  • Hospice or palliative care experience preferred.
  • Certification from the Academy of Certified Social Workers or Hospice and Palliative Social Work preferred.
