About The Position

Hartley House is a historic settlement house serving the Hell’s Kitchen neighborhood in New York City since 1897. Programs include after school and summer day camp programs for children, support services for neighborhood seniors. POSITION OVERVIEW: Under the supervision of the Assistant Director, the Social Worker is responsible for providing comprehensive case management services to clients while supporting the oversight and quality assurance of assigned Case Managers. This role works collaboratively with program leadership to ensure effective service delivery, regulatory compliance, and high standards of client care. The Social Worker also serves as Task Supervisor to MSW interns, providing structured guidance, documentation review, and support aligned with Council on Social Work Education (CSWE) competencies.

Requirements

  • MSW degree and experience working with older adults.
  • Knowledge of resource and benefit programs.
  • Excellent organizational, communication, outreach and advocacy skills and the ability to work as a team.
  • Computer literacy including proficiency in Microsoft Office applications.

Responsibilities

  • Preliminary screening of applicants for service at case intake, screening for program eligibility, conducting triage for emergency intervention, ascertaining short-term need, and providing accurate and appropriate information and referral services to clients
  • Conduct biopsychosocial assessments for new clients, coordinate and monitor services, including home care, medical and/or psychiatric care, etc.
  • Ongoing in-home visits to clients to evaluate client functioning, monitor risk, assess the status of current services, and update and revise the case plan; seeking appropriate guidance from the supervisor in situations requiring clarification and consultation
  • Assist with money management including monthly bill pay, budgetary assistance, and applying for private stipendiary assistance as necessary referral to, and maintenance of cooperative relationships with other community agencies, to meet the needs of clients
  • Eligibility screening for public benefits/entitlements and facilitating the collection of documentation to support these applications
  • Advocate on behalf of clients in matters relating to well-being such as tenant/landlord issues, legal affairs, and access to benefits
  • Maintain database records that document personal profiles, important contacts, and narrative accounts of interactions with clients and others crisis intervention when necessary
  • Attend community meetings for the purpose of conducting program outreach and trainings designed to build knowledge
  • Support clients in NORC with providing case management and case assistance ( Naturally Occurring Retirement Community)
  • Provide support, guidance, and oversight to assigned Case Managers to ensure effective case management services and adherence to program standards and funder requirements.
  • Conduct periodic case reviews with Case Managers to monitor service coordination, documentation quality, and risk management practices.
  • Serve as Task Supervisor to MSW interns, providing structured supervision, reviewing documentation, facilitating case discussions, and collaborating with academic field liaisons to support learning objectives
