What You’ll Do Be the Heart of the Team: Collaborate with clinicians and volunteers to craft personalized care plans that honor the patient’s "wants" as much as their "needs." Empower Families: From assessing psychosocial needs to navigating financial assistance and Medicare, you’ll be their champion and guide. Connect the Community: Act as a high-visibility ambassador, building bridges with local agencies to expand the reach of hospice philosophy. Requirements

  • A State Certified Social Worker with a deep understanding of the grief, loss, and the dying process.
  • Ready to hit the road (valid DL and insurance required) to bring care directly to those who need it.
  • Calm in a crisis and comfortable working in diverse home environments.
  • You possess world-class listening skills and the ability to educate staff and the community alike.
