About The Position
ATC Healthcare of Arizona is seeking a qualified Social Worker for a 13 week contract assignment at a large hospital in West Prescott, AZ . This role is ideal for a compassionate, adaptable professional who thrives in fast‑paced clinical or community settings and is committed to supporting patients and families through high‑quality social services.
Requirements
- Bachelor’s or Master’s degree in Social Work (BSW or MSW) from an accredited program
- Active Arizona Social Work license (or ability to obtain)
- Minimum of 1 year of social work experience (healthcare or facility‑based experience preferred)
- Strong communication, organizational, and advocacy skills
- Ability to adapt quickly to new environments and policies
Responsibilities
- Assess patients’ psycho-social needs and develop appropriate care or discharge plans
- Provide counseling, crisis intervention, and resource coordination
- Collaborate with interdisciplinary healthcare teams to ensure continuity of care
- Advocate for patients and assist with referrals to community resources
- Maintain accurate documentation in accordance with facility, state, and federal guidelines
- Support patients and families in navigating healthcare, social, and financial challenges
