About The Position

Social Worker – Skilled Nursing Facility Full Time Mary Ann Morse Healthcare Center, a 124 bed skilled nursing facility located in Natick, MA, is looking for a Social Worker to join our team. Mary Ann Morse Healthcare Center has been deeply rooted in the MetroWest community for over 25 years with a reputation for providing high quality care. This facility is situated in a beautiful setting with modern updates to the property and state of the art short term rehab unit and therapy gym. Summary: Work with Residents and their family members to identify their psychosocial, mental, and emotional needs and provide, develop and aid in access to services to meet those needs. Services will be provided in accordance with current federal, state and local standards, guidelines and regulations, our established policies and procedures, and as may be directed by the Executive Director, to assure that the medically related emotional and social needs of the resident are met and maintained on an individual basis.

Requirements

  • A graduate of an accredited School of Social Work, MSW, and LCSW required.
  • Skilled in communication, assessment and social work techniques.
  • Ability to work with a variety of disciplines in individual and team settings.
  • Working understanding of social systems and ability to implement state and federal regulations.
  • Experience working with individuals with dementia/cognitive loss and their families.
  • Must be able to show compassion, understanding, and patience when dealing with Residents.
  • Excellent organizational skills.
  • Ability to work independently.
  • Strong interpersonal /communication skills.

Nice To Haves

  • Five years’ experience with geriatric population, experience in licensed nursing home, and knowledge of MDS preferred.

Responsibilities

  • Contribute to and maintain high quality Social Work services that meet and exceed staff and resident expectations.
  • Maintain commitment to excellence in carrying out responsibilities.
  • Maintain focus on person centered care.
  • Develop preliminary and comprehensive assessments of the social service needs of each resident.
  • Develop a written plan of care (preliminary and comprehensive) for each resident that identifies the problems/needs of the resident and the goals to be accomplished for each problem/need identified.
  • Serve as active contributor in care plan meetings, discharge planning meetings, family meetings and team meetings.
  • Ensure collaborative relationship with residents and families to make informed decisions.
  • Coordinate the provision of social services to residents to improve the quality of life for individual residents and respond to issues identified by residents and families to promote satisfaction with services.
  • Review and revise care plans and assessments as necessary, but at least quarterly.
  • Participate in interdisciplinary care meetings.
  • Provide therapeutic interventions to assist residents in coping with transition and adjustment to the facility.
  • Refer residents/families to appropriate social service agencies when the facility does not provide the services or needs of the resident.
  • Participate in reviewing and developing facility social work policies concerning resident care and quality of life to assure compliance with state and federal regulations.
  • Provide information to resident/families of Medicare/Medicaid and other financial assistance programs available to the resident.
  • Understand and meet all state and federal requirements for social service documentation.
  • Assure that progress notes charted are informative and descriptive of the services provided and of the resident’s response to the service.
  • Prevent and address resident abuse as mandated by professional licensure.
  • Involve the resident/family in planning objectives and goals for the residents.
  • Discuss advance directives with resident and family, including Health Care Proxy, Living Will, and end of life decisions such as feeding tube, do not hospitalize, comfort measures only, DNR, hospice, and plan for funeral home.
  • Provide support and education to residents and families to assist in understanding of placement and facility issues.
  • Maintain confidentiality of all pertinent resident information.
  • Provide a high level of customer service to residents, coworkers and supervisor by being helpful and courteous and seeking and responding to input.
  • Demonstrate high interest in acknowledging and understanding others.
  • Provide feedback to Director of Nursing and/or Executive Director regarding resident satisfaction relating to the facility.
