About The Position

This full-time Social Worker position in the Case Management Department at Prime Healthcare St. Mary's Hospital in Kankakee, IL, involves providing emotional support, crisis intervention, and counseling to patients and families dealing with issues related to hospitalization, illness, disability, or trauma. The role also includes offering information and referrals for financial assistance and supportive services. Key responsibilities encompass case management, milieu management, completing psychosocial assessments, contributing to the development and re-evaluation of Treatment Plans as part of a multidisciplinary team, and coordinating discharge planning. The Social Worker is also responsible for maintaining necessary documentation and clinical records for each client throughout their stay.

Requirements

  • Bachelor’s degree Social Work, Marriage and Family Therapy or other related required
  • Current registration an intern with Board., as appropriate to education and licensing requirements

Nice To Haves

  • Minimum one (1) year Inpatient l or Outpatient Behavioral Health experience preferred
  • Current BCLS certificate upon hire and maintain current preferred
  • Masters in Psychology, Social Work, Marriage and Family Therapy, Licensed Professional Clinical Councilor or related field, Preferred

Responsibilities
