The social worker will provide psychosocial assessment and psychiatric diagnosis and treatment for older adult patients 65 years of age and older in the Strong Behavioral Health Older Adult Service. The social worker functions as primary therapist and is responsible for the overall coordination and implementation of the treatment plan for patients assigned to his/her caseload. The social worker participates on an interdisciplinary treatment team and develops liaisons with health and social agencies in the community, as well as with other health professionals within the hospital to support evaluation and treatment of the patient. The social worker also provides individual and group treatment, and crisis intervention as appropriate. The social worker may provide family treatment and case management services as indicated.
