About The Position

The Entertainment Community Fund (formerly The Actors Fund), a nationwide human services organization that helps all professionals in performing arts and entertainment, seeks a Social Worker to provide comprehensive and clinically focused case management and support services for clients of The Friedman to help them maximize their capacity for independent living. This includes primary intensive case management/counseling for residents identified as having the most significant psychiatric and/or substance abuse issues. This is currently a hybrid position.

Requirements

  • Masters in Social Work from an accredited school. LMSW required or obtain LMSW within first six months of hire.
  • Minimum of 2 years post masters experience working with HIV population, seniors and adults with mental health issues.
  • Strong verbal, communication and organization skills
  • Demonstrates strong clinical and engagement skills.
  • Ability to establish and maintain positive working relationships with staff and other agencies.
  • Proficiency in Microsoft Office (Excel, Word, Outlook, PowerPoint).
  • Ability to be self-motivated.
  • Work evenings on occasion.

Responsibilities

  • Manage clinical cases for caseload of residents receiving HRA HASA Supportive Housing Services in addition to seniors, low income working or disabled residents identified has having significant mental health and/or substance abuse issues.
  • Assist clients in maintaining medical, emotional and housing stability, utilizing counseling interventions and appropriate referrals and maintain a consistent schedule of face to face client contacts.
  • This includes orientation, interventions & counseling to address conflict resolutions when roommate conflicts arise.
  • Assist in the coordination of estate planning, including required legal documents such as power of attorney and living will; advocate for clients in obtaining and maintaining all eligible entitlements and services.
  • Maintain confidential up-to-date records, logs, release forms, and charts for each resident and assure the confidentiality of all information.
  • Responsible for maintaining documentation in assigned time frames.
  • Conduct intake social service interviews for new applicants which include applicant interviews & assessments/evaluations for appropriateness for shared living, roommate interviews, and program orientation to support services, and conduct initial assessment interviews with new residents, documenting a plan of service, established client/SW goals, and a time frame for follow up and review.
