Requirements
- Active Social Worker license (as applicable)
- Strong interpersonal and communication skills
- Ability to work independently and manage field-based responsibilities
Nice To Haves
- Experience in TBI (Traumatic Brain Injury) care preferred
Responsibilities
- Provide social work services to patients in the field
- Conduct assessments and develop care plans
- Collaborate with interdisciplinary teams to support patient care
- Maintain accurate and timely documentation
- Ensure compliance with all regulatory and company standards
