About The Position

Join the Inpatient Care Management team at the St. Cloud Hospital as a Social Worker. This Social Work role will be a part of the inpatient Care Management team but supporting patients that are scheduled for surgery at St. Cloud Hospital or CentraCare Plaza on coordinating post-surgical discharge plans and needs. Our Socials Workers at the St. Cloud Hospital serve patients and families in our community and beyond by helping them during a challenging time in their lives. Our Social Workers enjoy the variety of cases they are presented with as there is always something new and different. You'll collaborate with other Social Workers to gain knowledge and experience and work with an interdisciplinary team of nurses, therapists, physicians, and dieticians. You will have a designated patient caseload and will plan care to effectively meet the patient/family transitional needs including monitoring the length of stay and promoting community resources. We will provide supervision for new grads in-house or will reimburse up to $175/month for external supervision.

Requirements

  • Current Licensed Social Worker (LSW) in Minnesota required.
  • Bachelor’s or Master's degree in Social Work or Sociology.
  • At least two years of experience as a Social Worker, preferably in a healthcare setting.
  • American Case Management Association (ACMA) Case Manager-SW Certification required within five years.
  • To be eligible for the hiring and relocation bonus you must be an external candidate who is hired into an eligible full or part time benefited position.
  • Previous employees qualify if they have not worked for CentraCare for at least 9 months at the date of hire.
  • To be eligible for the relocation bonus, you must currently reside more than 50 miles away from St. Cloud Hospital and relocate to within 30 miles of it.
