About The Position
CentraCare’s Plaza expansion project is well underway and will be completed soon! We are looking to grow our team and add a dedicated Social Worker to support patients primarily within our Outpatient Rehab Clinic as well as providing additional support to primary care services across the St. Cloud area. As a Social Worker, you will play an important role in facilitating patient-centered care through a collaborative and comprehensive approach. Your work will include assessments, facilitating and advocating for options and services to meet individual health needs, discharge planning and much more!
Requirements
- Bachelor’s or Master's degree in Social Work
- Current Licensed Social Worker (LSW) in Minnesota
- A Valid Minnesota driver's license, a reliable vehicle available for work use, a driving record in good standing and the minimum amount of insurance liability coverage required by CentraCare
