Hours: The hours for this part-time position is 24 hours per week and the preferred schedule is 3 days per week.

Requirements

  • Masters in Social Work
  • Ohio Social Work license
  • Case Management Certification (CMC, ACM or ANCC) required or the ability to attain certification within 12 months of hire or within 12 months of eligibility
  • Three years of social work experience
  • Experience conducting and documenting psychosocial assessments and interventions to meet physical, functional, social, emotional, educationalneeds of the patient/caregivers
  • Experience in a Healthcare setting with community and national resource navigation and coordination, payment methodologies, mitigatingpsychosocial barriers to patient/family adherence

Nice To Haves

  • 5-7 years experience in a pediatric setting
  • Bilingual in English/Spanish

Responsibilities

  • The Social Worker is responsible for providing assessments, planning, interventions and follow-up for patients and family members at Shriners Children's across the continuum of care.
  • Through this function, by joining the team, you'll have the opportunity to make an impact on our patient's experience.
  • You'll help to provide continuity of care for positive outcomes in our family centered environment.
