The 29I Social Worker will identify and assess client and family needs and evaluate, coordinate, and provide necessary services and/or treatment. They will also provide in-home supportive counseling services to clients, complete required assessments, and assist individuals and families by helping to articulate goals and providing needed information. Primary Job Function Provide Behavioral Health Overlay Services (BHOS) interventions under the supervision of a licensed clinician while upholding the Glove House Standard of Care. Facilitate youth psycho-educational classes/groups, such as independent living skills, anger management, behavior management, etc. Provide supportive counseling services through individual, family (foster and biological) and group counseling. Provide case management services, coordinate services with other professionals and paraprofessionals and liaise with outside social service agencies and other organizations, where appropriate. Develop initial treatment plans within 30 days of placement. Complete intake assessments within 30 days of placement. These assessments include Bio-psychosocial, ACES, Sex Trafficking, CRAFFT, Trauma Screen, Individual Crisis Management Plans, and Developmental Milestones. Complete initial Developmental assessment within 45 days of placement. Complete annual updated trauma screens with youth as well as update ICMPS quarterly/as needed. Assist in Discharge Planning for youth and families. May monitor interns and/or volunteers. Provide training to foster parents and staff on relevant topics. Provide support in crisis situations through counseling, guidance, and de-escalation techniques. May transport clients to/from visitation or arrange for transportation to appointments, community agencies, etc. as needed. Provides support and supervision of other Social Workers across the agency. Reviews and approves necessary assessments as a licensed professional Fulfills designated leadership duties as assigned Supports the Foster Care & Bridges to Independence team and may be requested to provide other duties as requested by the Program Manager and/or Director of Programs, including potential Case Work duties Develop and maintain clinical records and program documentation, such as assessments, treatment plans, progress notes and discharge summaries, according to contract and Glove House standards. Complete necessary documentation in Connections and myEvolv following assessments, treatment plans, individual crisis management plans, therapy, and casework contacts in a timely manner Assist in creating and maintaining corrective action plans and safety plans for youth and foster families as appropriate. Contribute to an engaging, positive work environment. Participate actively in departmental meetings, training, and education, as well as the quality management process. Comply with Glove House policies, procedures, code of conduct, contracts, and other legal obligations. Demonstrate flexibility; perform other duties as assigned from time to time. Demonstrate the Glove House Values in the performance of all job duties.
