About The Position

This position is scheduled for 64-80 hours per bi-weekly pay period, working 8:00am-4:30pm Monday-Friday, with flexible days off. Responsibilities required, but not limited to: Supports the mission of Astera Health through personal conduct and behaviors that contribute ideas and suggestions, is helpful and friendly, demonstrates regular attendance and punctuality, and adheres to organizational policies and rules. Be a role model of Astera Health culture and core values. Serves as an integral part of the patient care team working with providers, nursing, and other clinic staff as well as the patient and their supports. Serves patients and their families across all Astera Health settings/facilities. Ensures patient self-determination in decision making. Ability to respond to multiple demands efficiently. Help patients, families, and outside agencies in the development of appropriate discharge plans that meet the social, medical, economic, and emotional needs of the patient. Assists in making appropriate referrals for patients such as long-term care, home care and community-based services. This includes follow-up and communication with other service providers. Working knowledge of area resources and processes for obtaining these services for patients. Serves as an active team participant in Utilization Review and the discharge planning process. Assists in planning and conduction of programs to assist patients with social problems and improve community health and counseling services. Assists in organization of support groups for patients and family members. Serves as the primary contact for swing bed referrals both internally and externally. Assists in completing initial paperwork as well as coordination of multi-disciplinary team meetings to monitor progress while in the hospital setting. Assists in making appropriate referrals for services upon discharge from swing bed.

Requirements

  • Bachelor’s degree in human services or related field such as Social Work, Psychology, Human Services, Counseling, or Sociology
  • 2 years’ experience in human services related work in hospital, nursing home, or county human services agency
  • Active license as a Licensed Social Worker
  • Ability to effectively operate standard office equipment and software
  • Strong oral and written communication skills
  • Demonstrates a high degree of commitment to customer service excellence
  • Possess cross-cultural sensitivity
  • Ability to work effectively with internal and external stakeholders, building strong relationships
  • Ability to perform tasks both collaboratively and independently
  • Demonstrates concern for patients of all ages, visitors, and co-workers.
  • Maintains confidentiality of information and privacy compliance.
