About The Position

The Social Worker assesses and plans interventions to assist patients and their families in coping with social, emotional, economic, and environmental challenges related to acute illness and hospitalization. Working collaboratively with the interdisciplinary healthcare team, the Social Worker conducts psychosocial assessments, coordinates internal and external resources, and facilitates safe and effective discharge planning. This role requires strong critical thinking, independent judgment, and the ability to function effectively in a fast-paced, high-stress clinical environment while ensuring compliance with treatment plans and regulatory standards.

Requirements

  • Bachelor's degree required; Masters preferred
  • Board Certified Social Worker, or related field (i.e. counseling) in the state where work is performed (CSW, LPCA, LCSW) within 6 months of hire
  • Handle With Care within 30 days of hire
  • Basic Life Support certification is required within 30 days of hire.
