About The Position

Our Social Workers work as a team collaborating with our internal medical and administrative staff to coordinate comprehensive care for our members. Providing clinical assessment and facilitating care plan management, the Social Worker will be the liaison between clinical providers facilitating medical care, psychosocial services and internal staff in order to ensure our members get total access to the resources associated with our program. The incumbent must be able to work independently and as a member of a team. They must be detail oriented and have strong interpersonal skills and written communication skills.

Requirements

  • Master's degree (foreign equivalent or higher) in psychology, counseling or related clinical field.
  • Current New York State Registered Nurse.
  • Valid NYS MSW license.
  • Two (2) years of full-time case management experience or clinical social work experience.
  • Mental health experience, particularly in post-traumatic stress, depression, anxiety disorders, and/or substance abuse disorders.

Nice To Haves

  • Four (4) or more years of full-time case management experience or clinical social work experience.
  • Experience with the first responder population.
  • Experience in administering clinical psychiatric diagnostic interviews and assessments.
  • Case management or medical social work experience specific to assisting cancer patients.
  • Case management experience specific to assisting patients with cognitive decline/dementia.
  • Experience evaluating suicidal ideation, using in-person, telephonic and telehealth modalities.
  • Experience with Microsoft Office.
  • Familiarity with Electronic Medical Records.

Responsibilities

  • Provide clinical assessment by reviewing self-report measures or administering clinical psychiatric diagnostic interviews, and if warranted, refer to community counseling. Please note this part of the job will require face-to-face interaction with our members.
  • Care/case management for World Trade Center Health Program (WTCHP) members with complex medical health conditions, mental health conditions, and/or cancer conditions.
  • Educate patients about WTC Health Program benefits and limitations.
  • Perform appropriate referral and disposition planning.
  • Put together prior-authorizations for services and authorize covered tests, procedures and treatments.
  • Put together higher-level authorization request for internal providers review.
  • Liaison between members, our providers, and external providers treating members.
  • Provide supportive services to members and families.
  • Assist with WTC Certification process and work closely with Certification team.
  • Uses clinical skills in supportive counseling for members dealing with mental health concerns.
  • Facilitates referrals to community resources.
  • Communicate with community agencies for the purpose of coordinating medical and psychosocial services; and provide information to other health care facilities to facilitate referral.
  • Education and assistance to help members understand and access program benefits, limitations, community resources.
  • Collaborates with other staff members to identify member needs. This includes collaboration between other departments within the organization.
  • Documentation in several databases including electronic medical record, and internal programs.
  • Other duties as assigned.
