Position Overview The Social Worker plays a key role in supporting the residents of Woodridge and their families as they navigate transitions in care and adjusting to their new environment. This role ensures each resident achieves and maintains their highest level of physical, mental, and psychosocial well‑being—while upholding all state regulations and CMS standards. This position is responsible for conducting assessments, participating in care planning, advocating for resident needs, and providing ongoing emotional and social support.
