About The Position

This full-time Social Worker position is within the Case Management Department at Prime Healthcare St. Mary's Hospital in Kankakee, IL. The schedule is for days, from 8:00 AM to 4:30 PM. The role involves providing emotional support, crisis intervention, and counseling; information and referral; and referrals for financial assistance and supportive services to patients and families with problems related to hospitalization, illness, disability, or trauma. The Social Worker will also provide case management and milieu management, complete psychosocial assessments, assist in the development and re-evaluation of Treatment Plans, work as a member of a multidisciplinary team, and coordinate discharge planning. Additionally, the role requires completing necessary documentation and maintaining clinical records for each client during their length of stay.

Requirements

  • Bachelor’s degree Social Work, Marriage and Family Therapy or other related required.
  • Current registration an intern with Board, as appropriate to education and licensing requirements.
  • Minimum one (1) year Inpatient or Outpatient Behavioral Health experience.

Nice To Haves

  • Current BCLS certificate upon hire and maintain current.
  • Masters in Psychology, Social Work, Marriage and Family Therapy, Licensed Professional Clinical Councilor or related field.
