About The Position

Make a difference in the lives of others by working as a Social Worker with CentraCare’s Home Health and Hospice team! As a vital member of our interdisciplinary team, the Social Worker plays a crucial role in facilitating optimal patient outcomes through a collaborative approach. This position involves assessment, planning, facilitation, and advocacy to address individual health needs effectively. The Social Worker will ensure the efficient and appropriate use of resources across the continuum of care while promoting evidence-based practices and compliance with professional standards. We are committed to providing compassionate care and support to our patients and their families. As part of our Home Health and Hospice team, you will have the opportunity to make a meaningful difference in the lives of those we serve while working in a collaborative and supportive environment. This position will primarily serve patients in St. Cloud, Sartell, Sauk Rapids, Foley and surrounding areas.

Requirements

Bachelor's or Master’s degree in Social Work
Current Licensed Social Worker (LSW) in Minnesota
Valid Minnesota driver’s license
a driving record in good standing
reliable transportation
insurance liability coverage following CentraCare’s policy
