The Social Worker position at Advocate Aurora Health involves providing comprehensive social work services to individuals or groups. This includes screening, psychosocial assessment, discharge planning, determining social work goals, and implementing appropriate concrete and/or therapeutic interventions with follow-up. The role requires a substantial degree of professional judgment to recognize deviations in patient/family functioning, anticipate problems, and maximize coping capabilities. Responsibilities also include documenting activities, advocating for patient involvement in care plans, coordinating interventions with healthcare teams and community agencies, and providing crisis intervention. The social worker will assist in managing hospital resource utilization for patients with complex continuity of care needs, support case management, and educate patients/families on Advance Directives. Effective communication with patients, families, healthcare teams, and community agencies is crucial for resolving issues impacting continuity of care. The role also involves maintaining professional relationships, adhering to the Code of Ethics, participating in staff orientation and educational programs, and contributing to quality improvement.
