About The Position

This position serves as the primary discharge planner for individuals who have been hospitalized for an extended period. This role will also require the ability to link individuals to appreciate services and resources in the community, complete assessments and referrals, and work collaboratively from a multi-disciplinary recovery perspective.

Requirements

• Master's degree in social work and counseling from an accredited college or university and eligibility to be licensed and two years of supervised experience in the area of assignment or two years of experience at the lower level social worker, License or position equivalent.

Nice To Haves

• Must have a Master's in Social Work (MSW), License-eligible
• Experience working with persons with Mental Illness and/or Developmental Disability
