Hours: Shifts varies: Sun, Mon, Fri, Sat | No Supervisory Responsibilities | No Travel | Pay commensurate with experience JOB SUMMARY: Coordinates the discharge process for patients leaving the hospital, ensuring a safe, smooth transition to the next level of care. This includes assessing post-discharge needs, coordinating with interdisciplinary teams, arranging services, and educating patients and families about care plans, resources, and follow-up requirements. Performs all work with accordance to the mission, visions, and values of Ashe Memorial Hospital (AMH).

Requirements

  • Bachelor’s Degree in Social Work field preferred.
  • Experience in Hospital or Health related field preferred.
  • High school graduate or equivalent.
  • Previous experience in social work preferred.
  • Must be willing to receive all required vaccinations (i.e., flu shot, etc.). All new employees working must be fully vaccinated as a condition of employment (unless the new hire has requested and received an exemption). Candidates for employment will be notified of this policy requirement prior to the start of employment. After receiving an offer of employment, new employees must provide proof of vaccination or request and receive an exemption before beginning work.
  • Criminal background check and pre-employment drug screen required upon conditional job offer.

Responsibilities

  • Works in close collaboration with physicians and other health care personnel in patient/resident evaluation and treatment to further their understanding of significant social and emotional factors underlying patient/resident health problems.
  • Talks with physicians/other departments on daily basis to help resolve patient/resident issues.
  • Helps patient and family through individual or group conferences to understand, accept, and follow medical recommendations.
  • Each patient has a working effective discharge plan and reviewed daily.
  • Provides service planned to restore patient/resident to optimum social and health adjustment within patients/resident’s capacity.
  • Meets with individual families on daily basis to education re: services available and how they can be obtained.
  • Utilizes community resources to assist patient to resume life in community or to learn to live within limits of disability.
  • Works with outside agencies each day to coordinate financial and social issues.
  • Participates in planning for improving health services by interpreting social factors pertinent to development of program.
  • Calls with outside agencies as needed.
  • Must be able to demonstrate the knowledge and skills necessary to provide care, based on physical, psychosocial, educational, safety and related criteria, appropriate to the age of the patients served in his/her assigned service area.
  • Reports weekly on all department activity to include age specific competency.
  • Works with families on daily basis to help alleviate problems with financing and acceptance on NHP.
  • Meets with patients/families daily to discuss needs.
  • Does follow-up visits to assure continuum of care.
  • Works daily with programs and social services organizations to decrease length of stay and improve department activities.
  • Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served on his/her assigned unit.
  • Must demonstrate knowledge of the principles of growth and development of the life span and possess the ability to assess data reflective of the patient’s status and interpret the appropriate information needed to identify each patient’s requirements relative to his/her age, specific needs and to provide the care needed as described in the Social Services UR/DCP departmental policies and procedures.
  • All activities are reviewed daily to assure needs are met of all patients, births through geriatric.
  • Performs other duties as assigned.
  • Uses time productively.
  • Adheres to the Studer AIDET customer services principles: Acknowledge, Introduce, Duration, Explanation, Thank You.
  • Attends staff meeting and inservices.
  • Adheres to attendance policy.
  • Uses computerized punch system correctly to accurately record working hours.
