About The Position

Under the direction of a Clinical Supervisor or designee, is responsible to provide social work services in determining and assisting in the resolution of social and emotional problems.

Requirements

Licensed (LCSW or LMSW) Required
Master’s Degree in Social Work
Computer literacy

Responsibilities

Establishes and maintains positive relationships with patients, visitors and other employees. Interacts professionally, courteously, and appropriately with patients, visitors, and other employees. Behaves in a manner consistent with maintaining and furthering a positive public perception of BronxCare Health System and its employees.
Contributes to and participates in the Performance/Quality Improvement activities of the assigned department. Contribution and participation includes: 1) data collection; 2) analysis; 3) implementation of and compliance with corrective/improvement activities; 4) membership on CQI teams; 5) consistent adherence to established performance standards; and 6) adherence to the specific rules and regulations of the B.C.H.S’s Safety & Security Policies, Risk Management, Incident and Occurrence Reporting, Infection Control Policies and Procedures, and Patient and Customer Service.
Provides social work services to patients/families: bio-psychosocial assessment and treatment; risk screening and assessment (e.g. suicide, violence, abuse), mental status examination, crisis intervention, psychotherapy of various modalities/types (e.g. individual, group, family, supportive, CBT, DBT, relapse prevention, etc.), patient outreach, treatment planning, discharge planning, and other related services as needed.
Documents all services in the patient record accurately and in a timely manner as stipulated by hospital policies and consistent with regulations/standards of the Joint Commission (JC), NYS Department of Health, New York State Office of Addiction Services and Supports (OASAS), New York State Office of Mental Health (OMH), New York City Department of Health and Mental Hygiene (DOHMH), Substance Abuse and Mental Health Services Administration (SAMHSA), Drug Enforcement Agency (DEA), and other regulatory agencies as applicable.
Carries an active caseload as stipulated by departmental policies.
Collaborates with inter-disciplinary team and other department staff in providing patient care and ensuring continuity of care.
Collaborates with Utilization Management (UM) and PFS (Patient Financial Services) staff to secure treatment authorizations from, and provide treatment information to, managed care organizations and other payors.
Submits billing documentation information as required.
Provides reports, record documentation, and statistics as required.
May be delegated responsibility for in-service training for social work students, and other trainees.
May serve as liaison within the department, hospital and to community agencies.
May be delegated to represent social work department in job related community meetings and conferences.
Perform other activities as shall be deemed appropriate in assuring services to patients and the functioning of the Social Work Program.
