About The Position

"Navitas Healthcare, LLC" is seeking a compassionate and experienced Social Worker for a full-time, 14-week contract assignment in Camden, Maine. This day-shift role offers a consistent weekday schedule and guaranteed 40 hours per week. The Social Worker will provide comprehensive psychosocial support, advocacy, and case management services to residents and their families within a healthcare setting.

Requirements

• Active Social Work license in the State of Maine (LSW, LMSW, or LCSW required)
• Bachelor’s degree in Social Work (BSW) required
• 1–2 years of recent healthcare social work experience preferred (SNF, Long-Term Care, or Acute Care)
• Strong knowledge of discharge planning, Medicare/Medicaid, and community resources
• Ability to work independently in a fast-paced environment
• Excellent communication and documentation skills

Nice To Haves

• Master’s degree (MSW) preferred

Responsibilities

• Conduct initial and ongoing psychosocial assessments to identify residents’ emotional, social, and environmental needs
• Collaborate with interdisciplinary care teams to develop and implement individualized care plans
• Coordinate complex discharge planning, including community referrals, home health services, and durable medical equipment
• Provide short-term counseling and crisis intervention to residents and families
• Advocate for resident rights and assist with advance directives and power of attorney matters
• Support residents and families with financial resource guidance, including Medicaid and insurance navigation
• Maintain accurate, timely documentation in compliance with state and federal regulations
