Social Worker
About The Position

Responsible and accountable for assessment of psychological, social, emotional, cultural and financial needs, and the resulting treatment that impacts patients' health and recovery. Provides professional social work services that utilize clinical knowledge to address the needs of patients and families during illness, crisis intervention, and post-acute discharge planning. Collaborates with multidisciplinary team to develop transition plan and address needs in a timely manner. Participates in program planning and development to meet the needs of customers within the entity, health system and community.
Requirements
- Less than 1 year Applicable Experience
- Licensed Masters Social Worker - Various
- Master's Degree
Nice To Haves
- Current Missouri MO LMSW or LCSW
- Prefer experience
Responsibilities
- Assessment of psychological, social, emotional, cultural and financial needs
- Treatment that impacts patients' health and recovery
- Provide professional social work services that utilize clinical knowledge to address the needs of patients and families during illness, crisis intervention, and post-acute discharge planning
- Collaborates with multidisciplinary team to develop transition plan and address needs in a timely manner
- Participates in program planning and development to meet the needs of customers within the entity, health system and community
- Participates in daily treatment team meetings.
- Assists with discharging planning, making referrals and assisting with transportation arrangements stationed in the ED to assist with ED holds
