About The Position

We our looking for a dynamic individual to join our Case Management team as a medical Social Worker. The Social Worker's primary role is to support our patients and their families in navigating the challenges that can arise from medical diagnoses, treatments, and healthcare experiences. This individual will provide crucial emotional support, counseling, and practical guidance to help individuals cope with the physical, emotional, and social implications of their health conditions. They will collaborate with interdisciplinary healthcare teams to ensure that patients receive comprehensive care that considers both their medical needs and their psychosocial well-being. They assist in identifying available resources, such as financial assistance programs, support groups, and mental health services, to help patients and families manage the emotional and practical aspects of their healthcare journeys. This role also advocates for patients' rights and ensure that their preferences and values are respected in medical decision-making processes. Hours/Shifts 80 hours biweekly, day shifts, Monday through Friday Compensation Please note that final compensation will be determined based on experience, qualifications and internal equity considerations. The hourly wage range for a BSW candidate is: $30.44-$40.93/hour. The hourly wage range for a LICSW candidate is $38.95-$49.87/hour.

Requirements

  • Requirements are representative of minimum levels of knowledge, skills, and/or abilities.
  • Bachelor’s degree required (from accredited school of social work preferred)
  • Current MN Social Work license (minimum licensure level-LSW)
  • Should have knowledge of public assistance programs, community resources, good interviewing techniques, and counseling skills
  • Must demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients we serve across the enterprise
  • Must demonstrate knowledge of the principles of growth and development over the life span
  • Must be able to assess and interpret data about the patient’s status in order to identify each patient’s age specific needs and provide the care needed by the (neonate, pediatric, adolescent, adult, and/or geriatric) patient group

Nice To Haves

  • Previous social work experience in a medical setting preferred
