As a Social Worker, you will coordinate the facility placement, community agency referral and social coping aspects of your assigned patients. You will complete an initial assessment and compile discharge plans reflecting clinical and social needs by collaborating with the patient, their family and medical staff. In the discharge placement options, you will educate patients and families about facility placement and work with community mental health centers. In this role, you assess patients’ suicidal/homicidal potential, take appropriate actions and implement seclusion and restraint procedures as needed. ACCOUNTABILITIES The licensed social worker demonstrates knowledge of the principles of growth and development, the ability to assess and interpret age/developmental specific data, make treatment recommendations, and evaluate patient/family response to treatment. Provides appropriate data to other caregivers and involves patient (patient centered care), family, significant others, and community providers in decision-making related to plan of care as appropriate , when indicated , and according to department/hospital policies and procedures. The patient population served includes: infants, children, adolescents, adults, and geriatric patients. The licensed social worker completes timely psychosocial assessments for patients admitted to the Coping Center and having a length of stay of 24 hours or more. The licensed social worker plans interventions and implements psychosocial interventions based on patient assessment of needs and strengths. Performs individual, couple, family, and/or group counseling for patients on the unit and elsewhere in the hospital, according to patient need. Utilizes evidence-based approach. Performs social psychotherapy under supervision according to social work scope of practice requirements. Assists in scheduling family, marital or significant other meeting with L.I.S.W.-S when indicated . Initiates discussions with the physician and treatment team regarding potential patient discharge plans. Facilitates patient discharge plans per approval of the multidisciplinary team. Demonstrates knowledge of community resources and assists with development of referral processes. Provides educational information re: community resources, coping skills, etc. to patients, families, and other staff. Documents patient observations, activities, and data collected in the electronic medical record and/or on paper forms per current process. Documentation is accurate , clear, legible, descriptive, using correct forms, terminology, and abbreviations. All entries are signed, dated, and timed. Errors are handled appropriately. Documentation reflects concise, efficient use of time. Documents timely individual, group, and counseling note on each session conducted. Provides assistance to facilitate the therapeutic transfer of patients from one hospital unit to another unit or to other hospitals/agencies. Completes authorization forms for release of patient information. Notifies case managers and/or prehospitalization care providers of patient's admission (with consent of the patient). Maintains communication with external care providers. Complies with HIPAA regulations. Participates as directed, in making post-discharge follow-up contact with patients. Attends and participates in a variety of meetings including multidisciplinary team, community, and departmental meetings as directed. Safeguards hospital supplies, property, and equipment from damage, misuse, or loss. Efficiently uses supplies and inventory. Understands scope of independent practice and performs with appropriate supervision . ProMedica Internal Only Reports promptly any problem related to patient care, or hospital activity. Reports any known or suspected improper business practices to the Hospital Compliance Officer or to any administrative staff member. Assists in the assessment of quality and appropriateness of Coping Center programs and levels of patient satisfaction. Participates in monitoring, evaluating, and modifying social work programs. Actively participates in hospital-wide quality and safety programs. Supports and builds teamwork/communication within the department and between other departments. Responds appropriately to visitors, patients, and public . Performs other related duties as assigned.
