About The Position

Regional West Medical Center is seeking a compassionate and dedicated Social Worker (BSW) to join our Cancer Services team. Reporting to the Director of Cancer Services, the Social Worker serves as a vital link to the community, offering case management, emotional support, and resources to help patients achieve and maintain maximum functioning.

Requirements

• Bachelor of Social Work (BSW) from a CSWE-accredited program.
• Certified Social Worker through the Nebraska Department of Health.
• Strong interpersonal and communication skills.
• Ability to work collaboratively in a team environment.
• Commitment to patient safety, integrity, and accountability.
• Proficiency in documentation and use of software systems.

Nice To Haves

• Previous experience in a healthcare setting preferred.

Responsibilities

• Conduct comprehensive psychosocial assessments and identify discharge planning needs.
• Develop and coordinate discharge plans in collaboration with patients, families, and the interdisciplinary team.
• Educate patients and families on available resources and options.
• Communicate effectively with physicians, ancillary staff, and serve as a resource for the healthcare team.
• Maintain accurate documentation of assessments, plans, and interventions.
• Advocate for patient rights, choices, and safety throughout care and discharge processes.
• Provide emotional support to patients and families during life-changing events.
• Comply with mandatory reporting laws for child or adult abuse/neglect cases.
• Perform other duties as assigned.
