Requirements

• Must have at least 2 years of experience working as a Social Worker.
• Master's Degree in Social Work preferred (MSW License is not required)
• Nursing or Rehabilitation Facility experience is a MUST!
• Should have a good working knowledge of available social services and other community resources in the area
• Must have excellent communication and social skills
• Self- directed but team-oriented

Responsibilities

• Counseling residents and family members; facilitating meetings
• Complete Admissions, Psycho-social Assessments, and Advanced Directives
• Make referrals for health services and coordinate discharge planning
• Completing all required documentation for medical records
