Requirements

Bachelor’s degree in Social Work from an accredited institution OR Bachelor’s degree in a related human‑services field (sociology, psychology, gerontology, rehabilitation counseling, special education) AND at least 1 year of supervised social work experience in a healthcare setting.
2+ years of experience working in a geriatric or long‑term care setting.
Strong communication skills — you’re great at connecting with all kinds of people.
Excellent organization and time‑management abilities.
Warm, empathetic, and grounded in professional boundaries.
A team player who brings positivity and collaboration to every interaction.

Responsibilities

Create and guide a Social Services program that truly supports residents and their families.
Build strong, friendly partnerships with teams across the community.
Help new residents find just the right place to call home.
Offer compassionate counseling to residents, families, and staff.
Keep documentation organized and meaningful — future-you will be proud.
Lead or support training that helps our teams grow and shine.
Stay on top of regulations and help keep us in tip-top compliance shape.
Support transitions between levels of care and keep families informed.
Join committees, projects, and initiatives that elevate resident care.
