About The Position

The Social Worker is responsible for providing psychosocial support to residents and their families, facilitating care planning, discharge planning, and ensuring residents’ emotional, social, and mental health needs are met in accordance with federal, state, and facility regulations.

Requirements

  • Bachelor’s degree in Social Work (BSW) preferred.
  • Strong interpersonal and communication skills
  • Knowledge of long-term care regulations and resident rights
  • Ability to manage multiple cases and work in a fast-paced environment
  • Compassionate, patient-centered approach

Nice To Haves

  • Experience in long-term care or rehabilitation preferred

Responsibilities

  • Resident & Family Support
  • Conduct psychosocial assessments upon admission and as needed
  • Provide counseling and emotional support to residents and families
  • Address adjustment issues related to illness, disability, aging, or placement
  • Advocate for residents’ rights, dignity, and quality of life
  • Care Planning
  • Participate in interdisciplinary care plan meetings
  • Develop and implement individualized care plans addressing psychosocial needs
  • Monitor and document progress toward care plan goals
  • Discharge Planning
  • Coordinate safe and timely discharge plans
  • Arrange community resources, home care services, and follow-up appointments
  • Collaborate with hospitals, rehab teams, and external agencies
  • Regulatory Compliance
  • Ensure compliance with CMS, state, and facility regulations
  • Maintain accurate, timely documentation in medical records
  • Assist with grievance resolution and abuse/neglect reporting
  • Psychosocial & Behavioral Interventions
  • Identify and address mental health, behavioral, or substance abuse concerns
  • Coordinate psychiatric services when needed
  • Support residents experiencing grief, loss, or end-of-life issues
  • Admissions & Transfers
  • Assist with admission interviews and social histories
  • Support residents during room changes, transfers, or discharges
  • Family & Community Relations
  • Serve as a liaison between residents, families, staff, and community resources
  • Educate families on resident rights, services, and care options
