Social Worker - Extended Care Facility (Per Diem)
About The Position
As a Social Worker, you play a pivotal role in providing essential support to residents, addressing their social, emotional, and related difficulties associated with medical conditions. Operating under the supervision of a qualified social worker, either through consulting or direct supervisory channels, this position involves independent action, contingent on daily supervision. Collaboration with professionals and external agencies is integral to providing assistance and guidance to patients. Key responsibilities include creating Comprehensive Care Plans, assisting in the admission process, maintaining information on community resources, and actively participating in the development and implementation of residents' rights and responsibilities.
Requirements
- Graduation from a regionally accredited or New York State registered college or university with a Master’s degree in social work, sociology, psychology, human services, or resources; or
- An equivalent combination of training and experience as indicated above, acceptable to the New York State Health Department.
- Must have experience in long-term care.
- Strong interpersonal and communication skills.
- Empathy and compassion in dealing with residents and families.
- Collaboration and teamwork with the Multi-disciplinary team.
- Attention to detail and organizational skills.
- Adaptable to changing circumstances and patient needs.
Responsibilities
- Provide social work services to help residents adjust to illnesses or disabilities, aiding in their initial facility adjustment and continual environmental adaptation.
- Independently create Comprehensive Care Plans and document changes supporting the Care Plan.
- Assist in the admission process, explaining procedures to residents and family members.
- Maintain information on community and health resources for residents' care and post-discharge.
- Participate in the development and implementation of residents' rights and responsibilities.
- Contribute to the process of transferring residents to other levels of medical care when appropriate.
- Record social information to develop resident case histories.
- Collect and maintain data from various sources to establish an overall approach to solving medical and social problems.
- Work with private, voluntary, and public agencies to identify the availability of medical and health-related community services.
- Assist residents in meeting diverse social, psychological, physical, environmental, family, financial, and related needs through coordination with the facility's Multi-disciplinary team and external resources.
- Provide care, treatment, and services to adults and geriatric patients as assigned.
