GENERAL PURPOSE: Provides professional social work services to patients and their families. Collaborates with hospital staff and community social services resources relative to the psycho-social problems of patients. Participates in formulating procedures and policies of the Division of Social Service. Participates in educational and research programs, including the orientation and training of new staff and students.

Requirements

  • The social worker must have an MSW from an accredited social work graduate program and must obtain New York State license within eighteen months of employment.

Nice To Haves

  • Prior clinical experience in a medical setting and supervisory experience preferred.
  • The social worker should be familiar with medical social work and community resources preferred

Responsibilities

  • Performs psychosocial assessments of patients and families referred by clinic providers or identified through high risk screening. These patients include, but are not limited to; elderly or disabled persons in need of home services or protection; persons experiencing emotional crises; and patients with new, chronic or terminal diagnoses and those to be evaluated for transplant or mechanical device.
  • Provides social work service to patients with problems identified through psychosocial assessments. Treatment may include: crisis intervention; supportive counseling; problem-solving; information and referral, respite or long-term facility based care; assistance with environmental problems. Services are largely provided within the hospital but may include home or agency visits if required by the patients' needs and situations.
  • Collaborates with other clinic care providers in development and implementation of interdisciplinary care plans.
  • Coordinates referrals to and acts as liaison to community agencies regarding services to individual patients.
  • Provides internal and external systems advocacy as indicated in care plans of individual patients.
  • Maintains current knowledge of community resources relevant to the clinic patient population.
  • Provides appropriate assessment and intervention services for children and adults suspected of being victims of abuse, neglect and/or domestic violence.
  • Handles risk management issues, in close consultation with his/her supervisor and the Clinical Manager
  • Documents social work activity in the electronic medical record
  • Provides social work coverage, as requested, to other clinical units or programs during co-workers absence
  • Completes the following Administrative Functions:
  • Completes Hospital and SW Division required in-service training programs and ongoing professional development activities including those that are designed to enhance knowledge and skills in protective work
  • Completes required Employee Health appointments within required timeframe
  • Attends all appropriate Social Work Division meetings including, but limited to – SW Division meetings, complex case discussion, transplant staff meeting, etc
  • Supervises social work students (MSW or BSW)
  • Orients and provides ongoing peer support to new social work staff
  • Other duties as assigned
