About The Position

Provides social service intervention for assigned caseload of long-term care residents, including documentation of residents’ emotional, psychological and physical status, attendance at care planning meetings, development and revision of social service care plans and MDS+ completion, assistance to residents and families in resolving issues and concerns and provision of assistance and/or referral for residents/families regarding financial matters related to their stay in the facility. Reviews admission policies and procedures with residents/families upon admission to the facility; secures signatures on relevant admissions forms. Arranges for and provides tours, as required, of the facility to prospective residents/families. Actively promotes and markets programs in the community under the direction of the Director of Social Services. Requires full access to all medical records, including those of current, prospective and discharged residents for purposes of care planning, MDS+ completion, resident/family education and continuity of care. Performs duties according to the highest ethical and moral standards, and in adherence with the facilitiy’s corporate compliance plan and all applicable governing laws. Adheres to the facility’s confidentiality policy in accessing and safeguarding protected health information (PHI) of residents and employees. Performs other duties as assigned by supervisor.

Requirements

  • Bachelor’s degree in Social Work, Psychology or related field and experience in a geriatric setting (may include college internships).
  • Knowledge/familiarity with Medicare, Medicaid and third-party insurance systems.
  • Strong communication and organizational skills, and flexibility to tolerate rapidly changing demands.
  • Personal computing skills.

Nice To Haves

  • Familiarity with electronic medical records (EMR) desired.
