Responsibilities: Assesses the environment of the home and community to identify elements which impact negatively on the patient/family ability to function. Provides direct short-term counseling services to patient/family or makes appropriate community referrals. Teaches coping skills to patients/families to assist them in their adjustment to the life changes associated with illness/disability. Participates in conferencing and planning with other disciplines caring for the patient. Prepares clinical progress notes and documents in the patients record, as indicated. Available for planned social work consultation on patient care with staff, as needed. Participates in evaluating overall position performance, goal setting and achievement, and performance improvement plan.
