Individual annual salaries/hourly rates will be set within the job's compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations. Responsibilities: The Social worker assigned to Pediatric Behavioral Health & Wellness (PBH&W): Brighter Days Urgent Care Center as both a Short-Term Assessment & Response Team (START) member and a Walk-In Clinician, functions as a therapist. As a START clinician, the social worker provides short-term solution-focused treatment to patients and their families and is responsible for assessment, crisis intervention, treatment planning, and therapy in the up to 6-session model of care. In the role of a Walk-In clinician, the social worker provides an initial psychosocial and mental health/safety assessment, and immediate evidence-informed crisis intervention, as well as follow-up recommendations and linkage to appropriate community resources and treatment (e.g., START).

Requirements

• Master’s degree from an accredited school of social work (CSWE) required
• Licensure as a social worker, as defined by the State Board for Social Work, State Education Department, Division of Professional Licensing Services is required. If licensure has not been obtained upon employment, the individual has 18 months within which to obtain it, unless duties will be performed under OMH then minimum of a limited permit (not to exceed 1 year) or NYS Licensure will be required upon employment start.
• Knowledge about and ability to provide behavioral health assessments/evaluations and evidenced based interventions required
• Commitment to and routine demonstration of biopsychosocial approach to care of individuals and families required
• Dedication to delivering a team-based integrated model of care required

Nice To Haves

• 2-3 years professional social work practice beyond the Master's degree preferred.
• Prior experience working in a psychiatric care setting and working with pediatric and adolescent population.

Responsibilities

• Provide direct patient care to youth and their families in the START (Short-Term Assessment & Response Team) track of the PBH&W Brighter Days Urgent Care Center under general direction and with considerable latitude for independent judgement. More specifically, provides short-term solution-focused treatment to patients and their families and is responsible for psychosocial and diagnostic assessments, crisis intervention, treatment planning, and therapy in the up to 6-session model of care. Mobilizes patient, family, and natural support networks when clinically indicated. Meets minimum target for clinical productivity.
• Provide direct patient care to youth and their families in the Walk-In track of the PBH&W Brighter Days Urgent Care Center. More specifically, provides an initial psychosocial and mental health/safety assessment, and evidence-informed crisis intervention, as well as follow-up recommendations and linkage to appropriate community resources and treatment.
• Documents treatment in patients’ electronic medical record in compliance with Program, Department and Hospital regulations. This includes the development of family centered, comprehensive and individualized treatment plans for patients with clearly defined therapeutic goals with patients and families; ensures that Social Work Division, Department, Program, and Hospital policies and procedures are implemented with regard to professional practice, patient care and discharge planning
• Coordinates appropriate referrals and linkage to mental health services as clinically indicated.
• Establishes and regularly maintains cooperative working relationships with referral sources and other professionals both internal and external throughout patient care activities (e.g., PCPs, community mental health agencies).
• Coordinates with professional and administrative staff of the Medical Center and University as well as support staff regarding patient scheduling, fees, and insurance issues.
• Participates collaboratively as a member of an interdisciplinary treatment team to provide coordinated care.
• Participates in routine committee process of the Social Work Division and the Child and Adolescent Clinic, and complies with Social Work Division and Departmental quality assurance and staff development requirements; participates in activities of the Social Work Division including, but not limited to timely completion of statistical reports/forms, time sheets and other Hospital or Division forms, Hospital and Division mandatory in-service training programs and UHS reviews, and Social Work Division and Program Staff Meetings; maintains professional CEU requirements by reading, attending seminars, workshops and conferences.
• Subscribes to the departmental goal of creating a culturally competent environment by treating patient, families, trainees, and co-workers in a sensitive manner with appropriate attention to cultural differences; participates in available cultural competence events and training appropriate to job duties.
